Here is where you will find license applications for Local, General, and Ordination; also the Insurance form, and the Pastor's Evaluation Form. License applications are provided in English and Spanish. The PDF formatted files can be downloaded and printed.
Local License Application
To be completed by the applicant. Signed by applicants pastor (if applicant is a pastor, can be signed by another UPCI Ordained minister). Applicant should take two copies when meeting with presbyter. Attach two photos of applicant and spouse to the application.

Pastor's Evaluation Form and Insurance Form
This form is to be downloaded and completed by your pastor. Your pastor should complete the form and send directly to your presbyter. The completed form should never be seen by the applicant.
